We encourage applicants to apply if they believe they have a continuity of service and have been licensed prior to the change of location, on or before August 1, 2021. Change of locations will be reviewed and granted the same opportunity as any other applicant.
Applicants must include their previous license tied to the old facility in the licensing application, where they are prompted to enter their license number and also upload their current and previous licenses.
