Is there specific information we need to put for our insurance to add CDSS as an additional insured? (Name, address of CDSS?)

The grant agreement requires commercial general liability insurance of not less than $1,000,000 for Centers and $100,000 for FCCHs per occurrence with an annual aggregate limit of $2,000,000 for Centers and $300,000 for FCCHs for bodily injury and property damage liability combined. All policies, except Workers' Compensation, shall be
endorsed to name CDSS as an Additional Insured with respect to the work to be performed under the Agreement.
