Is there specific information we need to put for our insurance to add CDSS as an additional insured? (Name, address of CDSS?)

For the House Title, right now it is under my name but are planning on getting a Living Trust this year. The title in the house will be changed to the Living Trust Name. Would this affect the process of the Grant Award?

The agreement states that we need to add CDSS under our home insurance. Do we add CDSS under the home insurance or under the business insurance?
